Database/Table php creation

Site www.athletesidentity.com. I am looking for creating of a database and tables to be connected to my site to make it function properly. I also request that this information be saved on a CD that is the program for the database and tables, which will make the sign up process work effectively and allow me to access and have fully functionable control over the administration page features. I need to have database created that works in conjunction with my website. It was originally created, but was destroyed by hackers. I need it recreated and connected to the site which is available on-line now, but members are not able to sign up and there is NO admin page. I need a database that enables users to sign up and create a profile. The profiles are based on number of days as an active member. All of that information is displayed on an administration page, which I can NOT access anymore either. That needs to be recreated and tied to the site. The administration page displays the members login name (e-mail) and password, number of days as an active member, whether they are active or inactive members, and gives me the authority to enable, disable or delete their profile and add or subtract days to their membership. Thus, only I am able to access the administration page which also allows me to manually create a profile for someone without having to have them become a member through the Sign-up process. Main problem is members can not sign up, and I can't access the admin page. (Nothing comes up when I go to [login to view URL]) Through a program that was destroyed, linking the administration page (where it displayed the number of days for each member to the site) there was a counter that automatically subtracted a day off each member's period at midnight of each day. So if a person signed up for a 3-month package, it would display 93 days. After the first day, the counter would automatically subtract one day and leave 92 Remaining Days on the admin page. Also, the database connected to the site allows an athlete to sign up and create a profile or Athletes Biography. They can't even sign up now because the database is empty. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? Buyer will receive exclusive and complete copyrights to all work purchased.
