Convert algorithm from pseudo code to working PHP code

Dear freelancers, I need a simple html site with some input fields and a button and a PHP script with an algortithm that will convert a curve into a polyline - within a given tolerance. Means high tolerance will result in many lines, low tolerance will result in few lines (see attached PDF containing pseudo code). You shall provide 1.) a PHP script containing an algorithm for "Piecewise Linear Approximation of Bezier Curves" - see attached PDF containing pseudo code 2.) a very simple HTML form to enter 9 values and a send button to send those 9 values to above PHP script and display the result as a list of numbers - no graphical output is needed - just list of numbers - no stylish design is needed for the HTML. Just basic HTML with maybe some style for formatting - The PHP script shall take following values: 4 points, each with x and y value plus 1 tolerance value. - Those 8 points are P0X, P0Y: starting point of a cubic bezier curve P1X, P1Y: First control point of cubic bezier curve P2X, P2Y: Second control point of cubic bezier curve P3X, P3Y: End point of cubic bezier curve tol: Tolerance (maximum distance) between curve and line segments (see pdf) Project must be finished within less than a week.
