Build a Web Form from Existing Database Schema.

We are looking to have built a web form for public order entry that connects to our existing database running on MSSQL. Freelancer will need to create and deliver an ASP.Net 4.0 application written in VB.net and all necessary SQL stored procedures for CRUD operations. Web form will accept one parameter upon load from either (POST) or (GET): Form_Type_Id. This Id will determine which form fields are shown. All form fields must be validated for proper data type and length. Some may require simple lookup dialog box. Freelancer will design a web form that collects information in several sections. This web form will need to calculate a simple total based on quantity and shipping. Tax and Item pricing is not required. Finally form will need to be submitted with a drawn signature. Full Scope document is attached as well as the data schema. Sample data base will be submitted upon acceptance.
