An offline webpage questionnaire that generates results in the following form: 1. Is s/he able to tell you his/her name(alert)? Yes No Unknown 2.(Male >50yrs or Female >12yrs) Did s/he faint(pass out)or nearly faint? Yes No / Unknown Not Applicable 3.(Female,12--50yrs) Is she pregnant? Yes No Not Applicable 4. Is there any vomiting? What does it look like? No Blood /Coffee ground Others Each combination will generate a different result. Certain options selected will prompt another sub-question E.g. If answers for questions 1 to 4 is yes, yes, no, yes, print result ABC. If yes, yes, no, no, print result XYZ Can be built in any language you deem fit, just needs to be offline and opened in HTML. I'm good with Html, CSS, Bootstrap , Javascript and Jquery. I can do this project in 2 days. Please specify more details. From the info you provided the user will select some answers to questions in a form and based on the answers we need to display some information and this project should be opened in a simple HTML page. If this is the requirement I can do this in 2 days including some styling to the page.
