Facebook App: Copy Simple JS calculator.

Hello, I have a simple energy savings calculator I would like to have duplicated or I need some type of plugin so I can use it as a facebook app. Basically the user enters in there cost per kilowatt hour and based on some stored figures in the attached files you get the answer.
