Web Script Needed

I need a simple web script where I can input people's name, height, weight and age. Then this will calculate his health condition whether he is over weighted or not. This can be done via PHP and flash can be used also. I will provide all necessary information to be set and also the host and database system. All you have to do is just to make the script.
