Email Form Page - VERY SIMPLE

ok. I need a simple form page - it will be 10 to 20 fields. the site is in ASP at the moment so would probably make sense to do it in the same thats it! it's a simple single page - so i dont want to see silly bids. i havent got time for it! it will need to fit into our site and look consistent. it will also need to send emails to specified recipients and if it doesnt cost any more, would be nice for it to add results to a spreadsheet or database, although this is not essential what IS essential is the price to be cheap and the turnaround quick. this HAS to be a reliable page.
