PHP Form

I just purchased PHP Forms and my hosting server uses mySQL.? I cannot get the forms to work on my server, so in addition to the form work below, I need you to make certain it all works on my hosting server.? I would give you access to all.? I use GoDaddy for hosting my secured site. I need a form created.? I will of course provide all questions and specify type of input response, i.e. text field, drop-down, radio buttons, or check boxes.? When a person submits the form, all infomation should be saved in mySQL database, an email should then be sent to me with the questions and answers, and an email should be sent to the respondent to confrim receipt of the form. You can then go through that form step by step, and then abort at the end, without submitting.? All info on my form would be very similar. ## Deliverables What I need is really just like this form I have just complete, but not nearly as much detail.
