Simple PHP Javascript Calender

Hi all, We have a form which 3 of the fields will be using a javascript calender. We have used this calender js on another form and it is working fine. However on our current form it is showing an error message. We would like the winning bidder to help us add the javascript to the three fields correctly. In the form we have marked the fields which needed to use the calender javascript. We need to get rid of the error message as well and ensure that when the user uses the field with the calender function and inserts data, the data is posted correctly into the database.
