Ajax Mysql Php Voting System

Hi Folk, i need a very simple voting script based on php + ajax/jquery + mysql see the attached files. details: as on image (index) there must be 3 sections - list with votes and % (max 5 itemes in list) + "show all" option. - pedestal with first 3 leaders - section with logos to vote how the vote system must work? on click logo action, the logo must hide and the submition form with fileds First name, last name and email must appear. once the user send a vote, the system must check for unique email. If the email exist --> show a sorry message "Sorry, this email has already voted", if not exist --> the system must send the verification email ( only smtp server) by clicking on verification link, if the email is ok then add vote to chosen item by user. Prevent from multiple verifying (if the user click again on verify link, the system must show a message " you have already verified your email) it is a pretty simple script and i will call this job "easy 20$" sugestion. write me a PM with a link to ready script once verified will pay immediatly via pay pal. thanx
