RSform and PHPScript for new form and invoice Admin email

I am looking for someone to create a form with RSForm, a Joomla Component. It is a registration form for events and it offer several purchase options. We have already created the form with all the fields required (see the attached pic). These are our needs: 1) Improve the graphical appearance of the form according to the template that we provide. 2) In addition to the captcha there is also a hidden field to improve the safety of the anti-spam, but must be verified if it works.
