image resize upload and save to mysql database

I need a image upload form with following features. 1.) image should be resized for width 700px and height according to width and should create a thumb 150x150x. 2.) image and thumb should save with unique file names. 3.) form should have a title field and it must be saved in mysql database alone with the image name and the thumb name. 4.) form should have ajax or jquery validation to check empty title, empty image field and image extension (allowed only JPG,PING,GIF and BMP) . please be fair while you're giving a price since this is not an big project.
