Customize a WordPress template

Hi, I have purchased a Wordpress template from Template Monster and will need it customized as follows: 1- Need to integrate logo 2- Change menu structure (6 menu buttons) 3- Create each individual page (6 pages) with the option of hiding some pages to start 4- Integrate a video on the front page 5- Add an opt-in form that is displayed on all pages 6- Hide all blog features (categories, archives, blog roll etc) 7- In page SEO 7- Some graphic design work for buttons etc if needed? I need this fairly quickly, ideally by Saturday June 13th. I need a clean professional job and you must have experience in setting up similar projects. Good English communication and fast response are a must. Preference will be given to developers who can make themselves available through phone or Skype.
