I need help to create a web-based survey and quiz to be used on an iPad. We have already created the design and this needs to be transferred to a working HTML survey. 2 of the 5 pages are attached. The pages should be written in HTML/CSS and use PHP for server-side activities and store data in an SQL database. Please also note the following: -This will be displayed on an iPad and should all be displayed on one page. - Booleon logic will need to be used as the final screen window depends on whether the delegate gave written feedback in the Quiz page 1 or in page 2 Question 4. In that case the second ‘inspirational’ screen will be used. - every quiz should produce a unique identifier number or letter code, that will be shown in the last screen window.
