Contact Form Plugin for WordPress

I would like to Create a plugin for WordPress, which allows web site visitors to email multiple people from one Form. The plugin will be called 'Contact All Form for WP' Please see the accompanying image, which I hope will demonstrate what is required - almost at a single glance. The multi-person contact form should be able to do the following: 1. Provide a standard email form to be displayed on a WordPress page, which asks for: Sender's email address, sender's name, subject and message. 2. The plugin should check that the email address supplied is in the correct format. 3. The message box should be as wide as possible, whilst allowing for the information that will need to go on the right-side of the page. If possible the message box dimensions should be adjustable by the site admin. 4. On the right-side of the page the plugin should display the names of the possible email recipients. The number of recipients that the admin can place in the list should be between 1 - 20. 5. In the case of narrow pages, there should be a layout option to place the recipient names and images above, or below, the contact form. Where the recipient names and images are to the right of the emailboxes (standard layout) the plugin should fit inside an 800 pixel page width. 6. There should be a checkbox against each name, in order to choose who should receive the emails. 7. There should be an option to add group emails. Example: Sales Department, Engineering Division. In this case, coma separated email addresses would be entered by the admin against a single reipient name in the list - Sales Department, for instance. 8. Email addresses for the recipients, must be added by the site admin and must not be visible to site visitors. 9. Against the recipients names there should be an option for the admin to include a 50x50 pixel image of the email recipients; or numbers, or bullets, or nothing. 10. If the admin chooses images and does not have an image for a particular recipient, a box will appear with a default image inside it. I will supply that image. 11. Each image should be (optionally) enclosed in a 1-3 pixel border. Border size should be selectable. 12. The image border colour should be selectable. 13. For each recipient there should be two lines of text. The top line will display the person's name, the bottom line will display their position within the organisation. Example: Shown in accompanying image. 14. The recipient's name and title text lines should have selectable colours and text weights (bold or regular). There should also be some control over the text size. 15. There should be a title line of text above the recipient list (as shown in the enclosed image). 16. The site visitor must enter a Captcha before sending the email. 16.5 When bidding on this project, please reply with the word 'Vixen', so that I know the specification has been properly read. 17. I would like 2 versions of the 'Contact All Form for WP' plugin. The standard version will only allow two names in the recipient list and will not include the Captcha. The Pro version will be as described, above. 18. I would like the tab for this plugin within the WordPress admin to be visible from the main menu bar on the left-side within WordPress. I do not want it hidden under the Settings tab. When the plugin is visible from the WP admin menu I would like the words 'Contact All' (without the quotes) to be displayed.
