Build a simple page: Map that represent routes on a map from 2 different tables.

I want a page to show routes on a map. We have a table with fields route_id, index, lat, lng. Index specify order of points to draw the lines. Data is taken from an app and from this website: [login to view URL] it is represent the density of traffic with colors the red, yellow and green lines give a general idea of how much traffic is on a road.
