Buyer agreement form

Hello, I have an HTML buyer agreement form that i need to do the following with it. 1- Upload in the server 2- integrate with an sms verification form 3- send a copy of the form to the buyer and myself in the specified email address 4- create a random code attach it to the form when sent to both of me and the buyer. i will provide the hosting and the sms / voice call service that we need to integrate verification code send.
