Convert PHP to Google Apps Script

Translate PHP functions to Google Apps Script. The page at [login to view URL] is used to make html forms. This page is written in PHP, HTML, and javascript. The code for this page is at [login to view URL] There are several "short codes" that this page uses to make the forms. These "short codes" are all described at [login to view URL] For this project, I would like a Google Apps Script that is attached to a Google Doc. The Apps Script would work this way: 1) The user types text and short codes into a Google Doc 2) "SOAPnote" menu item is clicked on a Google Doc 3) A form is displayed that works the same as it works on the page at [login to view URL] For testing, there is a library of files for testing at [login to view URL]
