create a survey

I need a survey to be written in php, where there will be 20 questions that are going to be timed, and they are multiple choice, enter answer, and enter multiple answers. I need the answer, the number of the person who answered, and i need it sent to a database. i would prefer if it was down through cloud 9, and in php, html5, and the database was SQL. I dont have the money to pay for it, as it is a small project, and i am only a high school student. if you are not interested please dont report, instead let somone else look through it, the survey questions will be inserted below. I need it before march 20. thanks.
