Modification of a information collecting script

I have a information collecting script. I want to have some modifications. First of all there are some css/design mistakes, which are not so huge, but they have to solve. Then i want to have a extra iframe at for requesting special informations at the end of the process. Moreover the requested information should be valided. After that all informations should collected in a web panel over mysql. You get the source code of the script and an explanation of the task with screenshots. Not more than 1-2 days.
