iPad survey app with drawing and signature capture

I need an iPad app that can do the following: 1. Run offline. Does not need connection to the internet to work. 2. Will store survey answers/ images/ drawings in a database that can be exported in a sql file or something I can reload on a desktop. (optional) 2. I need to be able to add/ change fields to capture data. If necessary I can list all the fields I need, but it would be a big plus if I can change them as I go versus having to get a new version of the app. 3. Besides capturing data entered into fields I need a space to draw. I would like to have an option to add stamps. Stamps= icon size images I predefine in the setup/ configuration page of the program. I can place these stamps in the drawing area. I have seen apps that will "correct" lines so even though I draw a squiggly line it will make it straight. I can also change the color of the lines and choose from different thicknesses among other options. 4. Capture pictures form the camera (iPad 2.) 5. Email the survey to two different emails. If we captured an email on the survey we can forward it. We can also define an email to forward the survey results to. If the device has no internet it will sync up as soon as it has an internet connection. 6. Generate a report based on the survey. The report will be generated as a PDF. We should be able to print it to a compatible airprint printer or attach it to an email (look at point 5.) The report will include all the information that has been captured in the fields, the drawing and the signature. The report will also include up to 9 pictures taken on a separate page. 7. Signature capture. The survey will be signed so we need to capture the signature. In a way it is a lot like a drawing.
