Server Alarms and performance issues

I need to have a way to know when the server is down, running out of memory or any of the services is not working properly. The environment is Ubuntu 16.04 and I need to have a dashboard (protected with login) to know what is the status of the website (displayed entirely not only ping) and chef if the services are down like apache server or MySQL and if there is a process that is taking more memory than expected and making the server run out of memory. I'd like to add some websites or services later and I should be able to do it by myself (I don't want to be reaching you every time). To keep track of this statuses I need to receive an email and/or a notification through any iOS/android app. I'm open the install the software that you recommend as long as it can fill my needs. When you finish you will have to show me what did you installed and which commands did you run since I don't want to lose track of what we have installed in the server.
