Website Design and Build

I'm in need of a designer/builder to build a website to support a business that runs competitions. The criteria is: First Page: Landing Page • Logo transition (logo and transition already created in mp4 ) • Once logo fades, landing page to indicate website is under construction. Section below to have contact us details (phone and email) Second page: The Player form • This page must be accessible via QR code and be mobile friendly. • Upon loading, the page will contain a form with various fields to be filled out by the competitor. A combination of pre-filled, free from and drop down fields will be required. The fields are defined and will be provided once the build commences. Terms and conditions need to be linked and accepted before the form can be submitted Upon submission of the form, redirect to payment gateway (Stripe?) to process payments via credit card or Applepay. Page 3: Database / Reporting • Need the ability to log entrant data from page 2 into a database . Data from player form should be logged and be able to be exported to excel for reporting purposes.
