Create web page to take input - run SQL - disply results

I need to run a query I have already against my database and then return results to a web page in a formatted page for printing. I have written a SQL statement and would liek to pass this to my SQL server. The statement will take 2 or 3 variables and apply them to the query. the query will run against my Database and then return results which will then be displayed using a format I have in a word Document. THe page layout will have to be made and the hosting folder will have to be set up. this page will be made availalbe to my intranet and not hosted on a regular web site, but only my intranet.
