Online Survey Software

Web-based survey software – similar to [login to view URL], [login to view URL] etc, but simpler, easier and quicker for the user to manage. Account creation - Activation by e-mail link - Payment options (cc, e-wallets). - Under one account can be several surveys. - Possibility to create a corporate account, under what users can be created (each user has basically its own account with its own surveys) Survey creation - A really simple, fast and intuitive survey creation functionality. Most common question types to be available (multiple choice checkbox, radio button, text area, drop-down list, matrix etc). Possible to reorder questions etc - Possibility to choose background template, add picture, divide the survey to several pages - Possibility to use a survey template (pre created survey) instead of creating the survey from scratch and modify the pre-created survey Survey publishing - link created for every survey - possibility to e-mail the link out to respondents as a personal invitation (possible to import or enter manually e-mail addresses) - possible to password protect the survey Responses follow up - To see how many have answered - To send reminder (maybe out of initial scope, but to send only the ones that have not responded – meaning that all the responders have their unique response link) - Receiving notifications of responses (after every response, on daily/weekly basis, when certain amount have answered) - Closing of the survey (survey cannot be answered any more) Analyze results - possible to export answers to Excel (csv) - possible to create a DOC/HTML/PDF/PPT report of responses (choose different graphs, enter additional comments) - possible to send out the report to responders The software will be in different packages – free, lite (10 questions and 10 responses), medium (up to 200 responses, e-mail and report generation possibility), pro (unlimited), corporate (with many users, period based fee) Detailed IT analysis is created will be given to the developer when the work starts. Would be good if the developer could also provide the visual design, but not necessary. PHP/MySQL preferred, code has to be well structured and commented – so additional features could be programmed later on also by other persons, as “advanced” package is planned to be created after the initial project. Preferred if no platforms will be used – so created from scratch.
