Require a form that submits the form data in a mySQL database table. Once the form is completed and submitted it should email the user a PDF certificate that includes all the information submitted in the form. The certificate should also display the end of warranty date which is one year from the purchased date entered in the form. If possible the emailed pdf should have a QR code that contains all the information submitted in the form.
