Need Simple cancellation page fixes with stripe and to stop SQL re-occuring

Hello freelancers I host a subscription based product it is handled by mysql and when people go to cancel they have to go to [login to view URL](subscribtion id here) But I would like to make it instead of it displaying Empty subscription ID! There was an error with your subscription cancellation. You did not pass subscription ID to this page on the page when there is no subscribtion ID it should display 4 text boxes that ask for username, email, payment method, and subscribtionID/OrderID and Comments(optional) and a submit button when they submit it should execute SQL to stop re-occuring 30days every month, also need it to send an email saying they've been cancelled and a confirmation ID. The cancellation system should work to cancel with my stripe just as if they visited the link by [login to view URL] Important: Need to have SQL knowledge and PHP
